What is Silver Diamine Fluoride (SDF)?

Silver diamine fluoride (SDF), also known as silver fluoride (SF), is a proven dental treatment used for decades to help stop and prevent tooth decay.

It combines silver ions, which kill harmful bacteria (anti-bacterial), with fluoride ions, which strengthen and remineralise tooth enamel. By disrupting bacterial biofilm and protecting weakened teeth, SDF offers a safe, cost-effective, and minimally invasive way to manage unrestorable cavities, particularly for young children, elderly patients, and those seeking a non-drill alternative.

Signs you might need Silver Fluoride

The two primary uses of Silver Diamine Fluoride are for tooth sensitivity and tooth decay. SDF is primarily used in children where restorations are not possible due to compliance or being a difficult area to restore; or the elderly where root decay is difficult to manage and seal effectively. Treatment with SDF may not eliminate the need for dental fillings or crowns to repair function or aesthetics and additional procedures may be done in tandem.

How is Silver Diamine Fluoride applied?

The procedure consists of drying the affected area, placing a small amount of SDF/SF on the affected area and allowing it to dry for at least one minute. It usually needs to be repeated 1 month after the initial application and may need to be maintained and applied twice yearly. It permanently stains areas of decay black.

Risks related to SDF or SF

It’s important to know the risks associated with Silver Fluoride treatment. Below are a few items to consider:

  • If you have an allergy to Silver, have painful sores or raw areas in the mouth, we do not recommend application of SDF
  • The areas of tooth decay will stain black permanently, however healthy tooth structure will not stain. Stained tooth structure can be removed if suitable and replaced with a filling or a crown later if required.
  • Tooth-coloured fillings and crowns may discolour if SDF/SF is applied to them. Colour changes on the surface can normally be polished off however the edge between a tooth and filling may keep the colour.
  • If SDF is accidentally applied to the skin or gums, a brown or white stain may appear that causes no harm and will disappear in one to three weeks.
  • You (or your child) may notice a metallic taste that will go away rapidly.
  • There is a risk that the procedure will not stop the decay and no guarantee of success is granted or implied.
  • If tooth decay is not arrested, the decay will progress. In that case the tooth will require further treatment, such as repeat SDF, a filling or crown, root canal treatment or extraction.

Alternatives to SDF include no treatment, which may lead to continued deterioration of tooth structures and symptoms may worsen. Depending on the location and extent of the tooth decay, other treatment may include placement of fluoride varnish, a filling or crown, root canal treatment or extraction.
